Scheme Overview

The scheme aims to provide accidental insurance cover to students studying in all Primary, Secondary, and Higher Secondary schools across Gujarat State. It offers financial assistance up to ₹50,000/- in case of accidental death or permanent disability to the student's parent or successor.

Eligibility Criteria

- The beneficiary must be a student studying in all Primary, Secondary, and Higher Secondary schools of Gujarat.

- The applicant must be the successor or the disabled beneficiary themselves in case of permanent disability.

- The applicant must appear in person with the original identity card.

- The applicant must submit the written application in the prescribed format to the local Nodal/Competent Officer.

- The application must be submitted within ninety (90) days from the date of death or accident.

Benefits

₹50,000/- per student.

In case of loss of two eyes or both limbs or arm/leg due to accident: ₹50,000/-.

In case of loss of one eye and one limb due to accident: ₹50,000/-.

In case of loss of one eye or one limb due to accident: ₹25,000/-.

The incident must be unexpected, sudden, and unintentional, and show visible signs of external violence.

The claim application must be submitted to the Nodal Officer within ninety (90) days from the date of the accident or death.

The benefit amount must not be paid for more than one category if the beneficiary is covered under multiple categories of the overall scheme.

17 Birth Certificate (Or Election Identity Card, only if the deceased/injured is illiterate and the Birth Certificate is unavailable).
18 Certified Copy of the Charge Sheet / Detailed Report Under Section 173 of the Code of Criminal Procedure (CRPC).
19 Dying Declaration.
20 Certified Copy of the Final Police Investigation Report Presented To the Sub-Divisional Magistrate (SDM) With the SDM's Order.
21 Certified Copy of the Postmortem Report.
22 Certified Copy of the Forensic Science Laboratory (FSL) Report (If Applicable, where chemical analysis of organs was sent).
23 Certificate From the Doctor Who Performed the Postmortem Stating the Cause of Death.
24 Final Assessment Certificate From the Medical Board / Civil Surgeon.
25 Postcard Size Photograph Showing Disability.

Step 1: Resolution by Insurer

Offline The Insurance Company or the State Insurance Fund must decide on the claim approval or rejection within sixty (60) days of receiving the application with complete and satisfactory information/documents.

Step 2: Claim Closure Timeline

Offline If the applicant fails to provide all requested documentation and fulfillments within the stipulated time, the claim will be closed automatically. The claim will also be permanently closed twelve months after the date of the accident/death, and no reconsideration request will be accepted thereafter. *If a claim is rejected, the beneficiary or their heir must first submit a representation to the designated Review Committee within thirty (30) days. *Legal action against the Review Committee's decision can only be pursued after filing the review, and the legal/quasi-judicial/judicial jurisdiction is exclusively in Gandhinagar. *Claim inquiries or representations must be made only by the applicant or beneficiary themselves, excluding any third parties like agents or lawyers.
