1. Players and their coaches from Uttarakhand who have achieved remarkable success at the national and international levels in the field of sports are eligible.

• Complete Details and Certificate of the Competition in which the medal has been won
• Permanent Residence Certificate of Uttarakhand
• One year sports diploma of the coach or equivalent or 06 weeks certificate
• Certificate of training of the National Association of the concerned sport
• Details related to the training of the coach on a stamp paper of ₹ 10 / - (by which the player has been regularly trained for at least 180 days or more a year before the medal win)
• Bank Account Number of the Coach
• Aadhaar Card
• National School Games medal winners by the School Education Department, World / All India University Competition / Championship medal winners by the Higher Education Department / University, Khelo India, Indian Rural Sports Competition / National Women Sports Festival medal winners by the Youth Welfare Department and All India Police Sports Competition / World Police / Fire Games medal winners by Uttarakhand Police Department It will be mandatory to get the sports certificates
